NIDDK and AHRQ convened four Technical Expert Panels (TEP) to identify data elements important to the care of people with multiple chronic conditions and associated data standards from common clinical terminologies (i.e., ICD-10, SNOMED-CT, LOINC, RxNorm, CPT). Each TEP is focused on data elements relevant to specific condition(s) included in the initial proof-of-concept project. The TEPs include the domains listed below. 

TEP Deliverable: Working Draft of Data Element Standard Set (DESS)

The TEP will identify the data elements required for electronically exchanging health information for individuals with T2DM, CVD and Pain with OUD. These data elements will inform the  development of the clinical information models.

The multiple chronic conditions (MCC) data elements & standards set (DESS) will include a prioritized set of data elements and associated standards from common clinical terminologies (i.e., ICD-10, SNOMED-CT, LOINC, RxNorm, CPT) identified by the project technical expert panels as critical to care for people with congestive heart failure, ischemic heart disease, hypertension, pain, opioid use, and/or type 2 diabetes. In addition, the MCC DESS will include data elements and associated standards identified through complementary projects, including the NIDDK CKD e-Care Plan project, the Electronic Long-term Services and Supports Service Plan project, the Gravity Project, the Pharmacy e-Care Plan, and PACIO.
